1. Step 1 · A bad note

   “Got stopped out again” doesn't count as a note.

   BTC 1h, Mar 18, 2024: a stop. “The market's against me” won't tell you anything a week later — it has no facts in it at all.

2. Step 2 · A good note

   A good note takes its facts from the chart.

   “Model entry at 67 775, stop beyond the sweep hit at 05:00, no rule broken.” This is −1R, not a mistake.

3. Step 3 · Template

   The template fits in one line.

4. Step 4 · A win with a rule break

   A rule break in a winning trade goes in the note too.
